Your Role as a Home Health Licensed Practical Nurse

As a Home Health Licensed Practical Nurse , you will:

• Deliver professional nursing services under the guidance of an RN, adhering to state regulations and CenterWell Home Health policies.
• Provide skilled nursing interventions tailored to the patient’s illness, rehabilitation needs, and preventative care, using a holistic approach.
• Utilize your clinical knowledge to facilitate problem resolution and achieve individualized patient goals.
• Collaborate with the RN Supervisor to adjust the Plan of Treatment as needed.
• Implement a systematic, goal-oriented nursing plan of care.
• Maintain timely communication with patients, physicians, and the healthcare team regarding patient needs and status changes.
• Uphold the highest standards of confidentiality and professional conduct.
• Ensure the availability of necessary supplies and equipment for effective care delivery.
• Document all required information accurately and promptly.

Required Experience and Skills

• Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.

Compensation and Work Schedule

Pay Range:

• $28.00 - $40.00 per visit/unit
• $44,600 - $61,400 per year base pay

Scheduled Weekly Hours: 40
